What we collect
We collect business entity data, state license numbers, contact details, and uploaded license and certificate-of-analysis (COA) documents submitted through Coral DBM™ intake forms.
How we use information
We use submitted information for formulary qualification, claims adjudication, and publication of qualified-product information in the Coral DBM™ dataset.
What is never published
Uploaded documents and contact details are never published. Documents are not published and are not shared with other participants.
Retention
We retain application records and supporting documents for as long as needed to evaluate qualification, maintain regulatory records, support active participation, and meet legal obligations. Records that are no longer required are securely deleted.
Sharing
Information is shared only as needed with Coral DBM™'s sponsor financial institution and plan participants. Uploaded documents are not shared with other participants.
Deletion requests
To request deletion of eligible application information, contact hello@coraldbm.com. Some records may need to be retained to satisfy legal, regulatory, security, or transaction-record obligations.
